In the event of the loss of hot water, the person in charge shall immediately contact the local health department.15A NCAC 18A .2815 (e ).Ms. R. stated that during her investigation, the kitchen door was observed open.”

Children were not adequately supervised at all times. At 4:02pm a toddler exited out of space #R2 and roamed the hallway alone, the toddler then entered into space #R4 (the classroom for three-year-olds). The classroom was empty due to the children being outside on the playground. A parent then entered the classroom at 4:06pm to pick up her child in space #R4 and noticed the toddler alone. She then picked up the toddler and returned him back to his classroom.
