Buildings and Grounds: (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.(1)The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement is not met as evidenced by: Based on observations and interviews, the staff do not keep the facility free of pests as rat droppings were observed in the classrooms: #2 and #7 under the sink areas and a dead cockroach in a trap in Room #2 under the sink area, which poses a potential risk to health, saftey, and personal rights to the children in care.

ALLEGATION(S): Facility did not maintain the center in a safe, sanitary and good repair to ensure the safety and well-being of children. INVESTIGATION FINDINGS: Licensing Program Analysts (LPAs), M.K. and S.S.Y. met with the Site Director, M.E., and explained purpose of visit- to deliver complaint investigation findings. During the course of the complaint investigation, LPA K. conducted observations and interviews. Interviews revealed there was a random smell in Room 2 bathroom. There was a urine smell at the start of the school year. Parents had raised some concerns and staff reported that they are getting children potty trained. On occasions, children have accidents and staff clean with paper towels. During this transition, the staff confirmed there was a smell but couldn’t clean with bleach while children are in care. This requirement was not met as evidenced by: The staff at the facility did not observe a child who sustained injuries while outside , which poses an immediate risk to the health, safety, and personal rights of children in care.

Unsubstantiated

ALLEGATION(S): Child's personal rights are violated by staff member. INVESTIGATION FINDINGS: Licensing Program Analysts (LPAs), C.N. and S.C., met with Site Director, M.E., and Head of School, R.V., and explained purpose of visit- deliver complaint investigation findings for the above allegation. LPA was admitted into the facility by office staff upon arrival. LPA N. conducted interviews with current and former staff members, the Site Director, and children attending the facility. No children interviewed expressed that their personal rights are violated by a staff member. Staff members and the Site Director did not indicate any concerns regarding staff employed at the facility. Only one former staff member expressed concerns regarding a staff member at the facility. LPA observed a classroom on 11/7/2023 and did not observe teachers violating children's personal rights. Based on the available evidence, it is concluded that although the allegation listed above may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. The allegation is thus UNSUBSTANTIATED. As a result of this investigation, no deficiencies were cited. Exit interview was conducted and report was reviewed with Site Director, M.E., and Head of School, R.V. A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Unsubstantiated Estimated Days of Completion:
