ALLEGATION(S): Staff do not provide adequate supervision resulting in inappropriate interactions between children. Director is falsifying incident reports. INVESTIGATION FINDINGS: On 07/08/2026, Licensing Program Analyst (LPA) K.H. conducted an unannounced complaint visit to deliver the investigation findings regarding the above allegations. Upon arrival, LPA met with Director S.K. and explained the purpose of the visit. It was alleged that staff do not provide adequate supervision, resulting in inappropriate interactions between children. It was also alleged that the director is falsifying incident reports. During the course of the investigation, LPA conducted unannounced facility inspections on 04/28/2026, 04/29/2026, and 05/28/2026, and observed that student to teacher ratios were in compliance with Title 22 regulations. LPA interviewed the head of school, the center director, and eleven staff members. LPA also interviewed parents. In addition, LPA reviewed the Employee Time Entry Report, student injury logs, and children’s records. Unsubstantiated Estimated Days of Completion:
