ALLEGATION(S): Staff caused multiple injuries to child in care taff spoke inappropriately to child in care INVESTIGATION FINDINGS: On February 11, 2026, at approximately 2:55pm, Licensing Program Analyst (LPA) M.O. conducted an unannounced visit to deliver complaint findings for the above allegations. LPA met with the Director A.O. and explained the purpose of the visit. Present during today’s visit was Director and 13 staff caring for 54 preschool children. Facility is operating within capacity. During the course of the investigation, LPA conducted both staff and parent interviews, conducted observations and reviewed pertinent documentation that was received. LPA determined that based on the information obtained there is not sufficient evidence to prove that Staff caused multiple injuries to child in care and Staff spoke inappropriately to child in care Although the allegation above may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore, the above allegations re UNSUBSTANTIATED. Upon receipt of this report, Licensee shall post the Notice of Site Visit. Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain postings as required will result in an immediate $100 civil penalty. This report is public and can be reviewed. Exit interview was conducted and report was reviewed with Director A.O. Copy of report and appeal rights were provided. Unsubstantiated Estimated Days of Completion:
