ALLEGATION(S): Personal Rights - Child sustained unexplained injuries while in care. INVESTIGATION FINDINGS: On 9/21/2026, Licensing Program Analysts (LPAs) A.Y. and T.D. conducted an unannounced complaint investigation. LPA met with Director, A.C. to deliver the complaint allegation listed above. Present for today's investigation were Director (10) staff, and 71 (children). LPA toured the indoor areas of the facility with the Director. During the course of the investigation, LPA interviewed the Director, staff, and parents. LPA conducted observations and reviewed supporting documents received. The allegation stated that child sustained unexplained injuries while in care. It was reported that scratch marks were observed on a child C1, and believed that it had occurred while the C1 was at school. Based on the result of the investigation, there were no indication that scratch marks had occurred while the child was at school. ***Continue Page 2*** Unsubstantiated Estimated Days of Completion:
