ALLEGATION(S): -Staff did not meet child's diapering needs INVESTIGATION FINDINGS: On 12/3/2025, at approximately 11:40AM, Licensing Program Analyst (LPA) J.T. conducted an unannounced complaint investigation visit at the facility. LPA met with Director, S.H., and explained the purpose of the visit. Present during the visit was the Director, 8 staff members, and 44 preschool age children. During the course of the investigation, LPA conducted site observations, record review, and interviews with relevant parties. The facility denied the allegation. LPA did not observe direct evidence to support or deny the above allegation. The preponderance of evidence standard has not been met, therefore the above allegation is found to be unsubstantiated at this time. No deficiencies were cited during today's visit on 12/3/2025. Appeal rights were provided and explained. A notice of site visit was provided and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director, S.H. ALLEGATION(S): Facility is operating out of ratio. INVESTIGATION FINDINGS: On April 4th, 2024, at approximatly 3:30 PM, Licensing Program Analyst (LPAs) J.G. and K.M. conducted an unannounced inspection in order to deliver findings on the complaint investigation for the above allegation. LPAs G. and M. met with the director, M.B. to discuss complaint allegations findings. Based on LPAs record reviews, and interviews which were conducted. The allegation may have happened or is valid, however there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Exit interview conducted and a copy of this report and appeal rights were reviewed and provided to director, M.B. Notice of Site visit was observed to be posted and shall remain posted for 30 days. No deficiencies were issued today under Title 22 Division 12 of the California Code of Regulations. NOTICE OF SITE VISIT WAS GIVEN AND SHALL REMAIN POSTED FOR 30 DAYS.
