ALLEGATION(S): Child sustained multiple injuries due to staff neglect INVESTIGATION FINDINGS: On September 2, 2026, Licensing Program Analysts (LPAs) M.M. and J.M.V. conducted an unannounced follow up Complaint Inspection. LPAs met with Site Director, M.M., and explained the nature of today's visit. LPAs observed a staff member supervising 22 napping children in Discovery Preschool Room. During the investigation, LPA M. interviewed staff and reviewed records. Based on interviews with staff, a child (C1) was never observed sustaining an injury while in care. Staff described one instance in which C1 arrived at the center with a scrape and a bump on the forehead; the parent informed staff that the injury had occurred at home. Staff further reported that C1 generally prefers to play independently. Interviews with staff also indicated that they have never observed any staff member using inappropriate discipline with children. Staff stated that they communicate with children calmly and utilize redirection as the primary method of guidance and behavior support.
