ALLEGATION(S): 1. Staff are operating the facility out of ratio. INVESTIGATION FINDINGS: On 8/26/2026 at 8:30 am, Licensing Program Analyst (LPA) M.E. conducted a subsequent complaint investigation. LPA met with the executive director, R.W., and explained the nature of the visit. On this visit, there were 7 Staff Members, 15 Toddlers and 17 Preschool Children. The facility is operating from Monday to Friday, 8:30 am to 5:30 pm. The Reporting Party (RP) alleges the above allegation. During the investigation visits, LPA conducted room observations, interviews and reviewed facility records. LPA also obtained copies of relevant facility records. In conclusion to the Departments investigation, the allegation is unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is deemed UNSUBSTANTIATED. ALLEGATION(S): - Staff are comingling daycare children. INVESTIGATION FINDINGS: Licensing Program Analyst (LPA) M.O. arrived to the facility unannounced to conclude investigation into the above allegation. LPA met with Director, R.W. Also present during today's visit were three additional staff members and 8 infant aged children. During the course of the investigation LPA conducted interviews, made observations and conducted record review. Based on interviews LPA received conflicting information regarding whether children are moved to the two's room or the infant/toddler room due to behavior reasons. Based on interviews conducted, the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the allegation is UNSUBSTANTIATED. A notice of site visit was given and must remain posted for 30 days. Appeal Rights were given and discussed. An exit interview was conducted. Unsubstantiated Estimated Days of Completion:
