Updated immunization records were unavailable to review for four student records reviewed. This was corrected during the inspection, when updated immunization records were obtained by the operation. Note - Immunizations records obtained outside of EMTRAK is required to have a doctor's signature/stamp.

Pre Employment Affidavit was unavailable to review for 5 of the staff records reviewed. This was corrected during the inspection, when the operation obtain completed 2912 forms from the staff.

There was sufficient evidence found during the investigation to show that the operation was experiencing issues related to dampness and humidity causing mold growth in one classroom. The operation has been in contact with air conditioning companies to remedy the issue.
