Drinking water was not accessible to the three groups of children for about 30 minutes during outdoor active play. This was corrected at inspection when the director took a pitcher of water and cups outside for the children.

Two electrical outlets in the outdoor play area were accessible to children and did not have a cover on them. This was corrected at inspection when the director added a cover to the outlets.

The operation's playgrounds still have climbing structures and slides, but they still don't have any loose-fill surfacing or unitary surfacing material in the use zones. They said that J. said they can have it this way. I explained that I had spoken with J. and she says that the grass must be removed. I showed them the use zones in which the grass must be removed.

Three out of the six children's records evaluated did not have an address for the emergency contact. The Second Director called all families and updated the file with the addresses.

It was observed that an outlet in the toddler bathroom and on the playground did not have safety covers. Second Director placed safety covers on both outlets.

It was observed that six toddlers 18 months- 24 months were watching an animated video. The caregiver turned off the video and read a book to the children.

Six bottles were observed. One bottle only had initials of the infant and four other bottles only had the infant's first name. The Second Director labeled all bottles with the infant's first name and first letter of last name during the inspection.
