An investigation found that a caregiver was using their personal phone to use FaceTime calls while caring for children. The caregiver also brought toddler children in her class onto the FaceTime call with her friend.

Supplemental Form 2936 created to show that the deficiency was corrected at inspection. This supplemental Form 2936 replaces the original Form 2936 left at the operation on 11/9/2021. During the walk through of the 2 year old classroom there was diaper cream in an unlocked cabinet accessible to children. This was corrected when the caregiver locked the cabinet.
