A DFPS investigator saw that there was a ladder leaning against the wall in the restroom used by children. The ladder could have fallen on the children. The ladder was removed from the restroom at the investigator's request.

A DFPS investigator found that the provider's adult son has been regularly present at the operation every morning without a background check.
