During the monitoring inspection it was observed that the loose fill for a 4 ft slide was only 2 inches deep. It is required to be 6 inches deep in the slide, use zone, area.

During the inspection, a caregiver was observed standing in the doorway supervising a group of children located inside of the classroom getting ready for nap and another group sitting in the hallway waiting to use the restroom. This was corrected at inspection when the caregiver had the children in the classroom go out and sit in the hallway with the rest of the children.

A review of records, one staff did not have documentation of the required 24- hour pre-service training. This was corrected at inspection when the staff added the additional proof of prior employment to the personnel record.
