746.2909(1) - Arrangement of Napping Equipment - Not Blocking Entrances or Exits · Medium High

Upon naptime there were one child that was laying in front of exit door. This was corrected onsite when the caregiver moved the child out from the exit door.

746.4503(a) - Individual Cot, Bed, or Mat for Each Walking Child Through 4 Years of Age · Medium

Upon observatation there were a total of nine mats that needs replacements. The nine mats had ripps and tear that displayed the form inside.

Two of the ten children records did not possess current immunizations records.
