746.4803 - Measurement of Use Zone for Stationary Equipment · Medium

There is a climbing dome on the playground that does not have a 6 foot use zone around all sides. There is a fence on one side and a tree on another side that the children can fall into. I spoke to the director about this.

746.5202(1) - Emergency Preparedness Plan - Evacuation, relocation, and sheltering/lock-down procedures · Medium High

The emergency preparedness plan was missing the following; name and address of an alternate relation site.

746.3407 - Maintenance of Building, Grounds and Equipment · Medium High

Observed the weed barrier sticking out on the playground which can cause a tripping hazard. I spoke to the director about getting more loose fill or removing the barrier so that it is no longer a tripping hazard.
