On 2/3/25, the director submitted trainings for one caregiver, one caregiver is no longer employed, and two caregivers are still missing the required annual trainings in emergency preparedness, administering medications, food allergies, and hazardous materials.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation · High

The director was present with an inactive background check. This was corrected at inspection when a new background check was submitted during inspection and the director was observed leaving until the background check is complete. HHSC will recommend an administrative penalty as a result of this citation, per HRC Section 42.078.
