Five caregivers were missing some of their required annual training hours for their last completed training years. Three people were missing all 24 hours of the trainings.

746.4805(a) - Measurement of Use Zone for Slides - in Front of Access and on Sides · Medium High

The operation has a slide that does not have proper 6 foot use zones on both sides of the slide.

746.4403(a) - Handwashing Sink in Diaper-Changing Area · Medium

The operation does not have a hand washing sink in the diapering change area for infants. The current sink that is used is located in a restroom and it does not allow the caregiver to continue supervising the classroom while washing hands.

746.1601 - Child/ Caregiver Ratio - 13 or More Children · Medium High

The infant classroom was not in ratio. There were 5 infants with 1 caregiver and the specified age group was 0 - 11 months.
