746.3805(b)(1) - Administering Medication - Original Container with Child's Name, Date · Medium High

Child's medication was not in the original container labeled with the child s full name and the date brought to the operation.

746.2907(a) - Alternate Activity for Non-sleeping Children · Medium

Two children ages 16 and 17 months were observed sitting in high chairs without an activity. One caregiver was observed patting a child to sleep while the other was observed playing with two infants that were awake in a center. When brought to the attention of the director, the children were taken out of the high chairs and given an activity.

There were 5 children that were sleeping that had their heads covered by their blankets. Note: This was corrected at inspection when the caregivers in the room quickly uncovered all the children's heads.

746.2428 - No Swaddling of Infant Unless Completed Sleep Exception Form on File · High

There was an infant that was swaddled in the crib, without having a Sleep Exemption Form on file.

There were three children files out of the six viewed that did not have updated shot records on file.

746.4907(d) - Installation and Maintenance of Loose-fill Surfacing - Proper Depth Maintained · Medium High

The loose fill around the slide exits were not the required 6 inches in depth. The ground was visible in these areas around the slides.

There were three children files out of the six viewed that did not have a signed health statement from their health care physcian in their file.
