Two children did not have a name, address, and telephone number of the child's physician and an emergency-care facility. The provider obtained the missing information during inspection.

747.801(7) - Records Maintained and Made Available - Safety Drills Documentation · Medium

The provider did not have safety documentation complete for testing the carbon monoxide detection system, smoke detector, and fire extinguishers monthly. This was corrected at inspection when the provider completed the monthly checks during the inspection.
