A vape pen fell from the pocket of a caregiver. It rolled on the floor, and a 20-month-old toddler picked up the vape pen, and held it out to her caregiver to take back.

It was determined during the course of this investigation that a caregiver did not check the label on a bottle, resulting in an infant receiving the wrong bottle.
