The investigation found that there is evidence that a caregiver spanked her own child at the operation. The caregiver also used threats of spanking while at the operation.

During the inspection, it was observed that an infant's hands were not washed following a diaper change. This was corrected when the caregiver was reminded to wash the child's hands and technical assistance was provided to both the caregiver and director.

Upon examining drills, it was found that fire drills had not been conducted monthly. This was corrected at inspection when one was done for the current month.

It was determined through the course of the DFPS investigation that a child in care was left outside alone for approximately 10 minutes with access to a wooded area, parking lot and main road.
